Please fill out all required data legibly. Be especially diligent regarding parent/guardian signatures and dates. Make sure you have included what grade your child will be in for the appropriate (SY) school year.
The phone numbers and addresses we require are for emergencies only. The information you supply will only be available in the Athletic Directors office. Each head coach of your child will have a copy of the physical for emergencies. This information will not be used for any other purpose, so please give us enough information to contact you in such an event.
